REGISTRATION

Families who live outside OPSD boundaries but wish to choice a student into the school will have additional steps to complete to be considered. If you are out of district, you must complete the OSPI choice request form via the link above.​​​

Students must be five years old by August 31 for the fall term they wish to enroll for.

Please be prepared to upload documentation for the following:

• Birth Certificate

• Immunization Records (or official exemption forms signed by provider)

• OPSD Health Information Form

• Home Language Survey

• WA State Race & Ethnicity Data Collection Form

• In-District Only: Affidavit of Student Residence (address verification in form of current gas or electric bill)
